The notebook is where failures are written down: what broke, the test that captured it, and what was changed because of it. The essays tell the story; the decision records below them keep the options that were rejected and what each choice costs.
E.01Why vector search alone failed usPure vector search missed a short note when the question used a different word form. The case became a test first; the fix was a second, fulltext retriever fused by RRF and a relevance filter that can answer “not in the knowledge base”. The first version of that fix — four-letter prefixes — failed both ways and was replaced by lemma + stem keys.E.02Permissions before the modelAccess control can live in the prompt, after retrieval, or in the query. A prompt is not a control and makes the model refuse entitled people; filtering after retrieval silently shrinks the results. So it lives in SQL — including the word statistics and the typo vocabulary, which would otherwise reveal what other people's documents contain.
